Our Services

Pilots and Air Traffic Controllers (ATC) with certain neurological conditions may require a comprehensive neurological evaluation as part of the FAA medical certification process. Aerospace Neurology Consultants PLLC provides specialized neurological consultation for FAA medical applicants (First-, Second-, and Third-Class) whose certification application requires neurological evaluation.

Our evaluations are conducted with consideration of current FAA aeromedical guidance and are intended to assist pilots or ATCs, and their Aviation Medical Examiners (AMEs) in providing the documentation necessary for FAA medical certification review.

The evaluation includes:

  • Detailed review of prior medical records and diagnostic testing

  • Comprehensive neurological history

  • Complete neurological examination

  • Review of neuroimaging, and neurophysiology studies (such as MRI, CT scans, EEGs, etc)

  • Review of relevant neuropsychological testing when applicable

Following the evaluation, a comprehensive neurological consultation report is prepared addressing the components required for the FAA comprehensive neurological assessment and relevant aeromedical considerations. The report is provided to the pilot for submission within the FAA medical certificate application submitted with their AME.

Our goal is to provide a timely, thorough, accurate neurological evaluation to support pilots and ATCs seeking a medical certificate.

Our services are limited to consultation. We do not provide ongoing neurological treatment or longitudinal care.

What should you bring to your assessment?

A thorough neurological evaluation depends on careful review of your prior medical history and documentation. Providing complete records in advance helps ensure that your evaluation is efficient, accurate, and aligned with FAA aeromedical expectations.

To assist with your evaluation, please provide all relevant medical records, including:

  • Prior neurological evaluations

  • Hospitalization records related to the condition

  • Clinic visit notes from treating physicians or specialists

  • Imaging reports (MRI, CT, or other neuroimaging)

  • Neuropsychological reports or cognitive testing results

  • Electroencephalogram (EEG) reports, if applicable

For imaging studies and EEG testing, please also provide a CD or digital copy containing the original images or recordings whenever possible, as the FAA may request review of the source data in addition to written reports.

Our evaluation will focus on reviewing and summarizing the medical records and information that are provided to us. The completeness of the evaluation depends on the accuracy and completeness of the documentation submitted by the applicant.

Under 14 CFR §67.403, applicants must not make fraudulent or intentionally false statements or conceal a material fact on any application for an FAA medical certificate. Applicants are responsible for fully disclosing their medical history on the FAA MedXPress Form 8500-8 and for providing supporting documentation when required as part of the medical certification process.

If obtaining records from hospitals or prior physicians is difficult, our office may be able to assist with collecting these materials as an administrative service for an additional hourly fee.

William Hoffman, M.D.

William Hoffman, MD is a board-certified neurologist with experience in both civilian and military aerospace and flight medicine. He has received specialized training in aerospace medicine and is a recreational pilot.

Dr. Hoffman attended Georgetown University School of Medicine, completed his neurology residency at Brooke Army Medical Center, and fellowship at Columbia University Medical Center New York, NY. He completed a two years postdoctoral research experience with the Federal Aviation Administration and is a previous Federal Aviation Administration Aeromedical Examiner (AME).

Dr. Hoffman has experience working with airline pilots and recreational pilots both simple and complex neurological histories. He is committed to providing clear, objective neurological assessments to support pilots and Aviation Medical Examiners (AMEs) within the FAA medical certification process.

  • After the evaluation, a detailed neurological consultation report is prepared. The report is provided to the pilot to share with their Aviation Medical Examiner (AME), who can submit the documentation to the FAA as part of the medical certification process. There is no guarantee that any applicant will be certified by the FAA.

  • This is very important!

    • All prior neurological evaluations

    • Imaging reports AND a CD of the images (MRI or CT scans)

    • Hospital records related to the condition

    • Neuropsychological testing results (if applicable)

    • Documentation requested by the FAA or AME

    • Neurophysiology reports AND the content on a CD (EEG, NCS/EMG)

  • The length of the evaluation may vary depending on the complexity of the neurological history and the amount of medical documentation that requires review.

  • A formal referral is not always required. However, many pilots are referred by their Aviation Medical Examiner (AME) or treating physician.

  • Consultations begin with an initial review of the pilot’s situation and relevant documentation. Please contact our office to discuss your case and determine whether an evaluation may be appropriate.

  • We work to schedule evaluations within a reasonable timeframe and make every effort to accommodate pilots as promptly as possible. Following the evaluation, we aim to provide a comprehensive neurological report as quickly and accurately as possible so that it can be submitted to your Aviation Medical Examiner (AME) for FAA review.

  • Yes. The FAA requires at least a one-time, in-person visit that includes a detailed neurological history and comprehensive neurological examination. This in-person assessment is an important part of ensuring that the evaluation meets FAA aeromedical expectations.

  • No. Aerospace Neurology Consultants provides neurological evaluations only. We do not perform neuropsychological, psychiatric, or psychological assessments.

    If these evaluations are required as part of the FAA medical certification process, pilots should work with their Aviation Medical Examiner (AME) to identify the appropriate specialist who can perform these assessments in accordance with FAA requirements.
